Foundational Community Supports (FCS) Program

Could you or someone you know benefit from Supportive Housing Services?

SideWalk has a team of knowledgeable and compassionate Peer Counselors who are ready to help. If you’re eligible, a SideWalk Peer can give you an extra hand to find a new place. SideWalk Peers are trained, experienced outreach and service workers.

What is the Foundational Community Supports program?

The Foundational Community Supports program (FCS) provides targeted Medicaid benefits to assist eligible individuals with complex health needs to obtain and maintain stable housing.

Supportive Housing Services Provide:

  • Housing assessments
  • Identification of housing resources
  • Support in obtaining a lease
  • Independent living skills development (i.e how to budget, use a washer/dryer, etc)
  • Landlord relationship coaching
  • Crisis management skill development
  • Connection to mainstream resources (holistic healthcare including medical, chemical dependency, in-home caregiving, behavioral health services, and income services)
  • Individualized collaborative plans for holistic wellness

Am I eligible for Supportive Housing Services?

Eligible populations for Supportive Housing services include individuals who are 18 or older and meet needs-based criteria and at least one risk factor:

Have at least one assessed health needs-based criteria and is expected to benefit from community support services:

  • Mental health needs where there is a need for improvement, stabilization, or prevention of deterioration of functioning resulting from the presence of mental illness
  • Need for outpatient substance use disorder treatment o Need for assistance with three or more activities of daily living (ADL)
  • Need for hands-on assistance with one or more activities of daily living (ADL)
  • Complex physical health need

Have at least one risk factor:

  • Are chronically homeless as defined by the U.S. Housing and Urban Development.
  • History of frequent and/or lengthy stays in the settings defined in 24 CFR 578.3, or from a skilled nursing facility as defined in WAC 388-97-0001:
  • History of frequent adult residential care stays
  • Have a PRISM score of 1.5 or above.
  • Note: This also includes individuals who previously met the U.S. Department of Housing and Urban Development (HUD) definition of chronic homelessness but have been housed in the last 60 days (Time housed may not exceed 60 days).
